iO TV: A digital cable box, HD digital cable box, or CableCARD (at an additional monthly fee) is needed for each television set to receive certain standard digital and HD services above Broadcast Basic and for some Broadcast Basic channels. QAM tuners can be used to receive digital channels in Broadcast Basic only and will receive some services on non-standard channel positions. Certain services are not available without a digital cable box. A digital cable box or HD digital cable box (at an additional monthly fee is required for each television set to receive interactive services, certain digital channels and international programming. Some On Demand titles at an additional monthly fee. Depending on where you live, some or all of your service and equipment rates, plus certain additional charges, may be subject to state and local fees of 0-5.25% and an FCC user fee of 8 cents, which will be added to your monthly bill. Number of channels received depends on your level of service and where you live.

OPTIMUM VOICE is a cable-modem service available exclusively to Optimum Online customers. If keeping your current phone number, a transfer charge may apply to residential accounts. Cablevision does not support use of Optimum Voice for emergency medical alert systems, all high security monitoring systems (UL 681 or similar) or fire alarm systems (UL 864 or similar). Customer must maintain alternate phone connection for such connections. The customer is solely responsible for contacting their central station monitoring provider to test and verify that their security system is in good working order. The customer is also responsible for any additional work required to ensure the proper operation of the security system. Without a battery backup, 911 service will not operate if power is lost (backup battery power supply is available at optimum.com/battery). If Optimum Voice service is disconnected, 911 service will be disconnected at the same time. Customer must provide notification to Cablevision before cable modem is moved to a different address in order for your 911 service to function properly. Call forwarding not available outside the continental US.
